For many of the people we work with at Brain Treatment Center NoVA, the problem is not a lack of effort.


The high-functioning child who holds it together all day and falls apart at home is trying.

The mom who manages the schedules, work, children and everyone else’s needs but feels constantly overstimulated is trying.


The veteran or first responder who cannot fully shut off the vigilance that once helped keep them alive is trying.


The executive who is productive but mentally exhausted, distracted or unable to sleep is trying. Sometimes, the issue isn’t knowing what to do. It is getting the brain and nervous system to become better at doing it. That is where neurofeedback becomes particularly interesting.


At Brain Treatment Center NoVA, serving Ashburn, Alexandria, Northern Virginia and the greater Washington, DC region, we are expanding our brain-based model of care to include BrainMaster neurofeedback. Neurofeedback gives us another noninvasive tool for helping individuals train patterns of brain activity associated with regulation, attention and performance. And importantly, we don’t view it as a stand-alone answer for everyone.

We view it as another piece of a comprehensive approach to brain health.


What Is Neurofeedback?


Neurofeedback is a form of EEG biofeedback.

Sensors placed on the scalp measure electrical brain activity. That information is processed by specialized software and translated into feedback the individual can see or hear, often through a game, video or other visual display.


The system is not putting electrical activity into the brain. Instead, it is listening to brain activity and providing feedback in real time. When targeted patterns occur, the brain receives feedback. Through repetition, the individual practices producing and maintaining desired patterns of brain activity. A simple way to think about it is:

The brain does something.
The system recognizes it.
The brain receives feedback.
The process repeats.


Over time, neurofeedback is intended to help the brain practice more adaptive patterns of regulation. This concept is based on learning and neuroplasticity: the brain’s capacity to change and adapt through experience and repetition.


Neurofeedback Is Not Just for Someone With a Diagnosis


One of the things we appreciate most about neurofeedback is that the conversation does not necessarily have to begin with:

“What is wrong with you?” It can begin with: “What is your brain having difficulty doing consistently?”

Focusing?

Transitioning?

Settling?

Sleeping?

Recovering after stress?

Maintaining emotional regulation?

Moving from high alert back into a calmer state?

Sustaining cognitive performance without becoming depleted?


Those patterns can show up across very different populations.

That is why neurofeedback may be considered for children and adults, from individuals struggling with attention or regulation to people interested in optimizing cognitive performance.


The High-Functioning Child Who Is Working Harder Than Everyone Realizes


Not every dysregulated child looks dysregulated. Some children perform exceptionally well academically. They are bright, verbal and capable. They may hold themselves together beautifully at school. And then they get home. The smallest request becomes overwhelming. Transitions are difficult. Frustration escalates quickly. Sleep may be challenging. Their brain seems unable to turn off. They may be anxious, perfectionistic, impulsive or emotionally reactive.


Sometimes parents tell us:

“Everyone says they’re doing great. But we see how hard they’re working just to keep it together.”


That matters. Behavior is not the only measure of regulation.

Neurofeedback provides another way to work with the patterns underlying attention and self-regulation by giving the brain repeated opportunities to practice more adaptive activity.

Research into neurofeedback for children has focused substantially on ADHD and attention. Some studies and systematic reviews have reported improvements in ADHD symptoms, including attention and executive-function measures. However, the overall evidence remains mixed, and neurofeedback should not be presented as a guaranteed replacement for established ADHD treatments.


At BTC NoVA, this fits naturally into our broader pediatric model. A child may benefit from neurofeedback while also needing occupational therapy, sensory-motor work, SPIN, functional health evaluation, behavioral or psychological support, educational interventions or other medical care.


The goal is not to make every child fit one protocol. The goal is to understand what that particular child needs.


For the Mom Who Is Always “On”


There is another population we think deserves considerably more attention.

Moms.

Especially the moms who appear to be functioning extremely well.

They are managing careers, appointments, school schedules, therapies, meals, relationships, household responsibilities and everyone else’s emotional needs.

They are functioning.

But their nervous system may rarely feel off duty.

You might recognize it as:

“I’m exhausted, but I can’t relax.”

“Every little sound bothers me.”

“My brain never stops.”

“I can’t focus like I used to.”

“I’m constantly overstimulated.”

“I finally get everyone to bed and then I can’t sleep.”

Sometimes the person who looks the most functional from the outside is using an enormous amount of internal energy to stay that way.


Neurofeedback may provide a different kind of training: not asking someone to simply “calm down,” but giving the brain an opportunity to practice patterns associated with regulation and cognitive control.


That does not mean every overwhelmed parent needs neurofeedback. Sleep, hormones, nutrition, medical conditions, psychological stress, environmental demands and other factors may all contribute to how someone feels. Which is precisely why we believe in comprehensive care.


Neurofeedback for Military Members, Veterans and First Responders


This population is particularly important to us.


Brain Treatment Center NoVA is veteran and Special Operations owned, and we understand that a nervous system shaped by operational environments does not always simply return to baseline when the mission ends.

Hypervigilance can be adaptive in dangerous environments.

Rapid threat detection can be adaptive.

Light sleep can be adaptive.

The ability to push through exhaustion can be adaptive.

Emotional compartmentalization can be adaptive.

The problem comes when the brain continues operating as though the threat is present long after the environment has changed.


For some veterans, military members and first responders, this can feel like:

  • Difficulty shutting the brain down

  • Poor or fragmented sleep

  • Irritability

  • Feeling constantly “on”

  • Difficulty concentrating

  • Emotional numbing or reactivity

  • Trouble transitioning between operational and family environments

  • Cognitive fatigue

  • Feeling uncomfortable when things become quiet

  • Difficulty recovering after stress


Neurofeedback is being studied as an intervention for PTSD and trauma-related dysregulation, with some research reporting encouraging findings. However, it is important to set appropriate expectations: current VA/DoD clinical guidance concludes that there is not yet sufficient evidence to recommend for or against neurofeedback as a treatment for PTSD. We believe that distinction matters.


For our military, veteran and first-responder patients, neurofeedback can be considered as one component of a larger treatment strategy when clinically appropriate. It should not replace evidence-based trauma treatment or necessary medical and psychiatric care.

Sometimes the question isn’t:

“How do we make this person less vigilant?”

It is:

“Can we help the brain become more flexible about when vigilance is necessary and when it can stand down?”

That is a very different goal.


Neurofeedback for Attention, Stress Regulation and Peak Performance


You do not necessarily need to be struggling significantly to be interested in brain training.

Neurofeedback has also been explored in areas involving cognitive performance, attention, mental fitness and self-regulation.

That makes the technology interesting for professionals, executives, athletes, students, military personnel and others whose brains operate under sustained cognitive demand.

A high-performing brain still needs flexibility.

The ability to activate when necessary is valuable.

So is the ability to recover.

At BTC NoVA, we are increasingly interested in that balance:


Activation when you need it.
 Regulation when you don’t.
 Recovery when the demand is over.


Why We Chose BrainMaster Neurofeedback


At Brain Treatment Center NoVA, neurofeedback is delivered using professional BrainMaster Technologies equipment and the BrainAvatar platform.

BrainAvatar is a professional EEG biofeedback, monitoring and imaging platform capable of processing EEG activity and supporting multiple forms of neurofeedback training. The system can display raw and filtered EEG waveforms, provide visual feedback and support individualized training approaches.


This matters because we did not want neurofeedback to become a generic wellness add-on.

We wanted technology that could integrate with the brain-based work we are already doing.

BrainMaster’s BrainAvatar platform is registered with the U.S. FDA for professional clinical use as a biofeedback, monitoring and imaging system.


That does not mean neurofeedback is FDA-approved to treat every condition discussed in this article. Neurofeedback research varies significantly by condition, protocol and outcome, and recommendations should be individualized.


Neurofeedback vs. MeRT: What’s the Difference?


This is a question we expect to hear frequently because Brain Treatment Center NoVA also specializes in qEEG-guided MeRT and TMS. They are not the same thing.


Neurofeedback is training.

The system measures EEG activity and provides real-time feedback that allows the brain to practice particular patterns.


TMS and MeRT are neuromodulation.

They use magnetic stimulation to influence neuronal activity according to a treatment protocol.


Neither should automatically be considered “better.”

They are different tools.


Depending on the individual, we may consider neurofeedback, MeRT or TMS, occupational therapy, SPIN, psychotherapy, functional health, integrative psychiatry or combinations of these approaches.


And sometimes the most appropriate recommendation is something outside our clinic.

That is what comprehensive care should mean.


Why We Don’t Believe in the “One Magic Therapy” Model


This may be the most important part of the conversation.

Brains do not exist separately from bodies.

And symptoms rarely exist in isolation.

Attention can be influenced by sleep.

Emotional regulation can be influenced by sensory processing.

Cognitive performance can be influenced by stress.

Stress tolerance can be influenced by metabolic and medical factors.

Behavior can be influenced by communication difficulties.

Trauma can influence the nervous system.

TBI can influence cognition, mood and sleep.

A child struggling behaviorally may need more than brain training.

A veteran with significant trauma symptoms may need trauma-focused psychotherapy.

A chronically exhausted mother may need her medical, hormonal, nutritional, psychological and lifestyle factors evaluated rather than having every symptom attributed to “stress.”

This is why we continue to build a comprehensive brain-first model at Brain Treatment Center NoVA.


Neurofeedback gives us another tool.

It does not eliminate the need for the others.


What Neurofeedback Can Look Like Within the BTC NoVA Model


Depending on the individual, a comprehensive plan may incorporate:

Neurofeedback to train targeted patterns of brain activity.

qEEG brain mapping to better understand measurable patterns of brain activity when clinically appropriate.


MeRT or TMS for individualized neuromodulation when indicated.

Occupational Therapy and SPIN to address sensory processing, regulation, motor patterns, functional performance and nervous-system integration.

Functional and Integrative Health to investigate relevant physiological contributors such as nutrition, metabolic function, gut health, hormones and other health factors when appropriate.


Psychotherapy to address trauma, cognition, behavior, relationships and emotional health.

Integrative Psychiatry when medication management or psychiatric evaluation is appropriate.


The specific combination should depend on the person.

Not the menu of services.


What Does Neurofeedback Feel Like?


Neurofeedback is noninvasive.

Sensors measure EEG activity from the scalp while the individual interacts with or watches feedback on a screen. Nothing needs to be injected, and EEG sensors themselves do not deliver electrical stimulation into the brain.

The experience can be surprisingly simple from the patient’s perspective.

The sophisticated work is happening behind the scenes as the system analyzes brain activity and provides feedback.


Neurofeedback also requires repetition.

Like learning any skill, meaningful brain training generally involves a series of sessions rather than a single treatment.


The number and frequency of sessions can vary based on the person, goals, protocol, response and clinical recommendations.


What Should You Expect From Neurofeedback?


We think expectations matter just as much with neurofeedback as they do with MeRT.

We cannot tell someone:

“This will fix your anxiety.”

“This will eliminate ADHD.”

“This will cure PTSD.”

“This will make your child regulate.”

Human brains are more complicated than that.

What we can do is identify appropriate goals, use objective information when available, train consistently, monitor how the individual is responding and adjust the plan when necessary.

Some people may experience meaningful improvement.

Some may experience subtle changes.

Some may not respond in the way we hoped.

And sometimes neurofeedback may reveal that another component of care deserves greater attention.

Good treatment is not about defending the tool. It is about paying attention to the person.
